I call this Bug-a-Form. It's just a beta so be nice.
I wasn't sure if this belongs in "random programs" or not. I mean it's more of an interactive web application then an actual executable program. I made a Form Making service. (if this is in the wrong forum, please move it)
This took me forever. I mean I started working on this during the big Jotform fad and I wanted a piece of that hype by making something superior to jotform. Tho now Jotforms are despised, I am still proud of it.

This sample falls way short of showing all the features, but I was too burnt out on debugging the javascript to put a whole lot of time into it.
Features include:
1. Text Input + validation for email, number, text, or no special characters
2. Textarea Input + optional min or max character requirement
3. Check Boxes + optional min or max check requirement
4. Radio Boxes + optional required radio
5. Select Menu + optional validation
6. Custom Redirection
7. Custom CSS
8. Custom Favicon
9. Custom Title (so people aren't aware of a form service when filling out your form)
10. A table view of the data logged by your form and you can remove entries.
11. I think that about covers it.
It's pretty simple to use, just set the setting for whatever you want to add like width, background color, background image, width, etc. and then click a spot on the grid to add it. Play around with the alignment and stuff.
